Admission criteria for end of life palliative care at La Maison Traversee

  • The person must be 18 years or older.
  • Be at the end of life stage with a prognosis of less than 3 months, as confirmed by a medical doctor.
  • The person must be a resident of the Laurentians (MRC Antoine-Labelle, MRC Laurentides, MRC Pays-d’en-Haut) or a member of his/her immediate family must be a resident of this Laurentian region.
  • Have a chronic terminal illness, regardless of the type of pathology.
  • The person knows that she is at the palliative stage of her illness and understands that there is no possibility of therapeutic healing nor will any treatments or efforts to cure or modify the progress of the illness be undertaken.
  • Level of medical intervention IV: interventions are limited to palliative care.
  • Prior to admission, the patient has the right to express his interest in eventually receiving medical help in dying, but understands that he can not be admitted solely for this specific service.

NB: For all admissions at La Traversee, if the patient’s health stabilizes or significantly improves as his stay nears or reaches the 3 month mark, the medical team, in collaboration with the patient/family, will re-evaluate his health status and make suggestions as to the best care option for him. For example, accommodation adaptations  or a return to his home with appropriate services as required by the patient and his family.

Admissions Process

Assessment of admission applications

  • When your request is received, the nursing care director at La Traversée will analyze your request.
  • She will contact the appropriate person to obtain more information if needed.
  • Your request will be presented to the admissions committee. It is therefore important to have on file as much information as possible.
  • Thereafter, the patient will be added to the waiting list based on priority criteria.
  • When a spot becomes available, the nursing care director will contact the person who made the request in order to organize the patient’s transfer on the date and time scheduled.
  • Please take note that admissions usually take place at approximately 9:30 a.m. or 1:30 p.m.
  • If you need to request an ambulance for transport, it should be scheduled for an 8:30 a.m. Arrival

Waiting list

  • When the number of residents on the waiting list exceeds the number of beds available at the home, we use a chronological priority criterion to determine the order of admissions. This criterion is the admission request date (if less than 3 months).
  • At any time, a resident who is in an urgent situation may take priority and would be admitted before their chronological rank.
  • A resident who is offered a bed but who does not yet feel ready to be admitted at that time, whatever the reason, will remain on the waiting list and will maintain their priority ranking while reviewing their decision to postpone the offer of admission within a period of less than 3 months

Visiting La Traversée

It is recommended for the terminally ill person or their caregiver to arrange to visit the home if it is possible for them to do so. The visit allows them to get a feel for the environment and reassures the terminally ill person and their caregiver. Please contact La Traversée nursing care director to arrange for an appointment to visit the home.

Cost

We are a non-profit organization and there are no fees associated with the services and care offered at La Traversée. However, incontinence underwear and dietary supplements such as “Boost” or “Ensure” are not included. There will be a charge for any medication that is not covered by your private insurance or by the government. Fees for meals apply only to relatives accompanying patients.

What personal effects to bring?

You can download the list of personal effects to bring by clicking on the button below. If you would like to sleep in the patient’s room, a folding bed will be available.

Transportation

If you are hospitalized, the hospital will organize your transportation to the home according to your level of autonomy. If you are at home, the nursing care coordinator will assess with you the best way (either with the help of a loved one, by ambulance or a medicar, if necessary).

Visits

Visitors of all ages are allowed at all times. The number of visitors is limited to 3 at a time, and a maximum of 4 people will be tolerated in the room when the resident is nearing the end of life.

It is up to the resident to determine the limits, if necessary.
