Admission Criteria
- The person must be 18 years of age or older.
- Being at the end of life with a prognosis of less than 3 months, as confirmed by a doctor (less than 2 months for chronic illnesses).
- The person must be a resident of the Laurentians (Antoine-Labelle RCM, Laurentides RCM, Pays-d’en-Haut RCM), or a family member involved must be a resident of the Laurentians region.
- Having a chronic terminal illness or terminal cancer, regardless of the type of condition
- The person knows that they are in the palliative phase of their illness, meaning that there is no longer any therapeutic prospect of a cure and that, therefore, no treatment or diagnostic tests will be attempted in the hope of curing the illness or altering its course.
- Level D medical intervention: interventions limited to palliative care.
- Prior to admission, the patient has the right to express an interest in potentially receiving medical assistance in dying, but may not be admitted solely for that purpose.
Note: Wandering behavior is an exclusion criterion. Being a resident outside our service area is not an exclusion criterion, but priority is given to people within our service area.
Note: For all admissions to La Traversée, if a person’s condition stabilizes or improves significantly to the point where their stay approaches or reaches 3 months, or if wandering behavior develops, the medical team—in consultation with the patient and family—will reassess the resident’s health and refer them to a more appropriate setting, such as: a transfer to another care facility or a return home with support services tailored to the needs of the patient and their family).

Admissions Process
Evaluation of Applications for Admission
- As soon as we receive your application, the Director of Nursing at La Traversée will review it.
- She will contact the person in charge to obtain additional information if necessary.
- Your admission application will be presented to the admissions committee. It is therefore important to have as much information as possible on file.
- Next, the patient will be added to the waiting list according to the prioritization criteria.
- When a spot becomes available, the DSI will contact the referring person so they can arrange the patient’s transfer on the scheduled date and time.
Waiting list
- When the number of residents on the waiting list exceeds the number of available beds at the facility, a first-come, first-served criterion is used to determine the order of admissions. This criterion is the date of the admission application. (If less than three (3) months.)
- At any time, a resident facing an emergency situation may be admitted on a priority basis ahead of their chronological ranking.
- A resident who is offered a bed but does not feel ready to be admitted at that time, for whatever reason, remains on the waiting list and retains their priority ranking until they reconsider their decision to defer the admission offer within a period of less than (3) three months.
Tour of the MSPLT
If possible for the patient or their loved ones, we recommend visiting the facility. A visit allows you to get a feel for the environment and reassure the patient and their loved ones. To visit the MSPLT, simply make an appointment with the DSI.
Cost
Since we are a nonprofit organization, there are no fees for the care and services provided at La MSPLT. However, incontinence briefs and nutritional supplements such as “Boost” or “Ensure” are not included. In addition, you will have to pay for edication as you would at home, meaning you will have to pay the amount not covered by your private insurance or the government. Meal charges apply only to family members who accompany patients, if applicable.
What personal effects to bring?
You can download the list of personal items to bring by clicking the button below. If you’d like to sleep in the resident’s room, there’s a folding bed. Just bring your own sheets and blanket.
Transportation
If you are hospitalized, the hospital will arrange your transportation to the MSPLT based on your level of independence. If you are at home, the nursing coordinator will work with you to determine the best option (either with the help of a family member or via ambulance or adapted transportation, if necessary).
Visits
Visitors of all ages are allowed at all times. The number of visitors is limited to 3 at a time, and a maximum of 4 people will be tolerated in the room when the resident is nearing the end of life.
It is up to the resident to determine the limits, if necessary.
