stabilizes or improves significantly to the point where their stay approaches or reaches 3 months, or if wandering behavior develops, the medical team\u2014in consultation with the patient and family\u2014will reassess the resident\u2019s health and refer them to a more appropriate setting, such as: a transfer to another care facility or a return home with support services tailored to the
